Where the real form lives
The current form is on socialregistry.wb.gov.in. Fill it online — that's the primary channel, and it validates your entries as you go. Paper forms surface at government camps and offices; only accept them from official staff, never from "agents" selling forms (they're free).
Filling it correctly
- Name and date of birth: exactly as on your Aadhaar. Mismatches are the #1 cause of delays and rejections.
- Address: your permanent West Bengal address as per supporting documents.
- Bank details: account number and IFSC, double-checked digit by digit; the account must be Aadhaar-seeded for DBT.
- Mobile number: the one linked to your Aadhaar — OTPs go here.
- Family details: household members as required; keep it consistent with your ration card.
After submitting
